The documents required along with your application depends on the type of funding you’re requesting and the way you apply. Pick your method below.

Disclosures and terms of service
Please review the following disclosures before sharing your information:
Business Documents
Financial and corporate records for your business entity.
- Articles of incorporation/organization, bylaws/operating agreement, and SS4 form (Federal TIN letter)

Two forms of ID for all individuals

2 months of bank statements (including account analysis if applicable)

2 months of merchant card processing statements

3 years of year-end business federal tax returns with all schedules and K-1s

3 years of year-end accrual financial statements (balance sheet + P&L)

Year-to-date accrual financial statement (balance sheet + P&L)
Personal Documents
Required for all guarantors, owners, and signers.

Personal Financial Statement

3 years of personal federal tax returns, signed and dated, with all schedules and K-1s

Unless assets/income are sole and separate, both spouses must initial and sign
Additional documentation
Investor property financing:
- Current rent roll/tenant schedule
- Copy of lease agreement(s)
- Most recent 3 years’ income statement of the subject property
Personal trusts:
- Certificate of Trust or copy of Trust Agreement
- If the Trust files a separate return: 3 years of federal tax returns with schedules and K-1s
- Current Trust financial statement
SBA loans:
SBA loans require several unique forms and documents. Please contact us at 480-358-1000 for details.
Constructions loans:
- Copy of plans, specs, and detailed cost estimate
- Information on the development/subdivision
- Information on the lot take-down process (if not already owned)
- Current work-in-progress report
